If you or your child are a client, we will further collect date of birth, genders, residential address, emergency records, health insurance numbers, patient treatment notes, and reports. You have the right to gain access to the information held about you or your child at Seed to Sprout.

We do use Cookies for web applications to tailor operations to your needs. You are able to turn off cookies to disable this. Through using Cookies, we do not in any way access your computer or personal information about you, it is only to help provide you with a better experience and visit on our website.

The platforms we use to collect your information is secure, with data encryption and maintained technology to protect your personal information.
